To apply for services, please plan to attend one of our orientation sessions held each Tuesday from 2:00 - 3:00 p.m. at our San Jose office. A reservation is not required however if you require any accommodations please contact us at least one day in advance so that we can make necessary arrangements. Prior to attending you may want to download and complete an application form and disability verification form to bring with you. A writtent verification of disability completed and signed by your practitioner is required to determine eligibility for services.
